Florida Hospital Heartland Division is committed to positively influencing the Heartland community and fostering the growth and health of our neighbors. Even in tough economic times, we still wish to help provide support when possible.  We are committed to being responsible with all of our resources, and sometimes we may not be able to provide the donation you have requested. 

Please keep in mind that, as a non-profit organization, we are also always looking for ways to raise funds to grow and improve our facilities and the care that we provide. 

We welcome your requests, but ask the following:

1. Please make your request at least 8 weeks prior to your event or sponsorship deadline.
2. Completion of this form does not guarantee acceptance by FHHD.
3. We are unable to accommodate requests for individuals, for-profits, and out-of-area organizations.
