Florida Hospital Heartland features a new Patient Assistance Program which helps insured/underinsured patients with high copays and deductibles.

1. The program applies to the following outpatient services:
Outpatient Surgery
Diagnostic Imaging
Laboratory Testing
2. Insured/underinsured patients with income under 400% of the Federal Poverty Limit are eligible for the program.
The patient or responsible party will need to answer questions relating to his/her income, as well as provide documentation/proof of their income to determine eligibility for this program.
